Tarjeta sanitaria: how to register with Spain's public health service
Spanish public healthcare is among the strongest in Europe, but one card stands between you and a doctor: the tarjeta sanitaria. Without it there is no family doctor, no referrals and no subsidised prescriptions. Here is who is entitled to the card, in what order to obtain it and what to do if you are not entitled to it yet.
- Entitlement to public healthcare comes from employment or autónomo status (alta with Seguridad Social), and also from legal residence in Spain.
- The order of steps: NIE and padrón, confirmation of entitlement through the INSS, then registration at the centro de salud for your address.
- The card is issued by your autonomous region: in Valencia it is the SIP, other regions have their own formats, so check your region's portal.
- If you are not entitled to free healthcare, the convenio especial is available: benchmark 60 euros a month under the age of 65 and 157 euros above it, with no cover for medicines.
- Emergency care (urgencias) is provided to everyone, but scheduled care is not available without the card.
How Spanish public healthcare works
The Spanish health system (Sistema Nacional de Salud, SNS) is funded from taxation and run at the level of the autonomous regions: each region has its own health service, its own portals and even its own card design. The basic principle is set out in law: after the Real Decreto-ley 7/2018 reform, access to the system is built around universality, covering everyone who lives lawfully in the country.
The patient's entry point is the centro de salud, the health centre for your address. There you are assigned a family doctor (medico de cabecera) or a paediatrician for your child. The family doctor handles most matters and issues referrals to specialists and for tests. Going straight to a specialist, as you would at a private clinic, is not possible in the public system.
What links you to the system is precisely the tarjeta sanitaria, your personal health card. It identifies you at reception, at the pharmacy it gives you prescription medicines with the state subsidy, and electronic prescriptions run on it too.
Who is entitled to public healthcare
Entitlement to healthcare, which is the condition for getting the card, arises on several grounds:
- Salaried employment. The employer registers your alta with Seguridad Social, so you are covered from the first day of the contract, and family members can be included as beneficiarios (dependants).
- Autónomo status. The self-employed pay contributions and obtain the same entitlement as employees.
- Legal residence. Residents without a job confirm their entitlement through the INSS (Instituto Nacional de la Seguridad Social) on the basis of lawful residence.
- Temporary protection. Ukrainians holding temporary protection status get access to public healthcare on the same footing as residents.
- Pensioners and benefit recipients, through their status with Seguridad Social.
A separate category is holders of residence permits for which private insurance was a visa condition (no lucrativa, for example): for them entitlement to free public healthcare usually arrives later, along with employment or a change of status.
Step-by-step registration: from NIE to card
The route is almost always the same, and only the regional details differ:
- NIE and legal status. The foreigner identification number is the basis for every procedure. How to get it is covered in our article on the NIE and TIE.
- Padrón. Registration at your address with the ayuntamiento (empadronamiento). It is the padrón that determines which centro de salud you are assigned to.
- Seguridad Social number. If you are employed or registered as an autónomo you already have one; if not, it is issued at a Tesoreria (TGSS) office.
- Confirming your healthcare entitlement. The document confirming your insured status is issued through the INSS (in person by appointment or online with an electronic signature).
- Registering at the centro de salud. With your passport, NIE, padrón and INSS document you go to the health centre for your address and fill in an application.
- Receiving the card. A temporary document with your number is issued immediately, and the plastic card arrives by post, benchmark from a couple of weeks to 1 or 2 months.
Practical advice: book every appointment in advance and keep your document set consistent. A mismatch between the address on the padrón and on the application means another visit.
SIP and other cards: every region has its own
There is no single nationwide card: it is issued by the health service of your region. The meaning is the same, and only the name, design and online services differ:
| Region | Card | Where to apply and check |
|---|---|---|
| Valencian Community | SIP (Sistema de Información Poblacional) | Centro de salud; portal san.gva.es |
| Catalonia | TSI (CatSalut) | Centro de atencion primaria (CAP) |
| Madrid | Tarjeta sanitaria (a virtual one is available) | Centro de salud; portal comunidad.madrid |
| Andalusia | Tarjeta sanitaria (Salud Andalucia) | Centro de salud; ClicSalud+ |
If you move between regions the card has to be reissued: a new padrón, a new registration, a card from the new region. Emergency care meanwhile works across the whole country regardless of which region issued the card. Check the details on the official health portal of your own region; for residents of Valencia, for example, that is san.gva.es.
Convenio especial: healthcare by subscription when you are not entitled
For those with no entitlement to free healthcare (no job, or a status that does not provide cover) there is the convenio especial de asistencia sanitaria, a formal contract with the health system for a fixed monthly payment. The basic terms are set by Real Decreto 576/2013:
- benchmark contributions for 2026: 60 euros a month for those under 65 and 157 euros from 65 upwards;
- you need documented continuous residence in Spain (or the EU) of at least 1 year and a padrón in the region where you apply;
- it covers the basic SNS package, but prescription medicines are not subsidised and are paid for in full.
The convenio is arranged through the health authority of your region. For many people it is a transitional solution: a year on it, then a move to ordinary cover through Seguridad Social once they start work.
When you need private insurance
Private healthcare in Spain does not replace the public system, it supplements it. There are three typical situations where a private policy is unavoidable:
- A residence permit requirement. For no lucrativa, student and several other statuses you need a private policy with no co-payments and full cover across the country.
- Speed. Waiting lists for specialists in the public system run to weeks and months, while a private policy cuts the wait to days.
- Dentistry and extras that the public system barely covers.
Choosing such a policy is a task in itself, with its own traps (waiting periods, exclusions for chronic conditions). We work through them in our insurance service and select an option to match your status.
How Krepko helps
Registering with the health service is a chain of four institutions: the ayuntamiento, the TGSS, the INSS and the centro de salud. Krepko walks it for you:
- We book every appointment and prepare the applications and document set: NIE, padrón, Seguridad Social number.
- We obtain the confirmation of entitlement through the INSS and complete the registration at the centro de salud, including children and dependants.
- If you are not entitled to free healthcare, we calculate which works out better, the convenio especial or a private policy, and arrange whichever you choose.
- The legally significant steps are performed by licensed specialists: lawyers, gestores and sworn translators.

Frequently asked questions
Emergency care (urgencias) in Spain is provided to everyone, regardless of status or whether you hold a card. But scheduled appointments, referrals to specialists and subsidised prescriptions are not available without being registered with the system. That is why it is worth getting the card as soon as you have the padrón and your healthcare entitlement.
Yes, a tarjeta sanitaria is issued to each person individually, newborns included. Children are usually added as beneficiarios (dependants) of a working parent through the INSS, after which the centro de salud assigns the child a paediatrician.
The card is issued by the autonomous region, so when you move you need to register a padrón at your new address and sign up with the local centro de salud, which will issue you a card from the new region. Emergency care across Spain is available with any regional card.
Mostly no. The public system covers a limited set: emergencies, extractions and specific programmes for children in some regions. Treatment and prosthetics are paid for privately or through private insurance, which is why many residents keep a policy as a supplement.
The plastic card usually arrives by post within a few weeks, with a benchmark of up to 1 or 2 months depending on the region. At the moment of registration you are given a temporary document with your number, which already lets you book appointments and get prescriptions.
